Appeals

Faculty of Pharmacy Appeals Process

In accordance with the Academic Board Resolutions (Student Appeals Against Academic Decisions) the Faculty of Pharmacy has established the following procedure for Student Appeals.

  1. Informal resolution with teacher (or UoS coordinator) - students are encouraged to approach UoS co-ordinators as soon as possible to resolve any issues.
  2. Informal approach the Associate Dean (as Dean's nominee) - if a student is not staisfied with the outcome achieved through the UoS co-ordinator they should approach the Associate Dean to seek resolution.
  3. Formal appeal to the Associate Dean (as Dean's nominee)
Informal approach the Dean - if a student is not staisfied with the outcome achieved through the Associate-Dean (Undergraduate) they should contact the Dean (via email in the first instance) to seek resolution.
  4. Formal appeal to the Dean
  5. Student Appeals Body - if the matter is not resolved at Faculty level students are able to submit an appeal to the Student Appeals Body

The student appeals policy can be found at http://www.usyd.edu.au/senate/committees/appeals.shtml

Student Code of Conduct

All students are expected to to follow the Student Code of Conduct which can be found on Policy Online. It can be downloaded here.

Attendance Policy

Attendance at Laboratory Classes, Tutorials and Clinical Placements is compulsory for all students unless it is explicitly stated in the syllabus document that attendance is optional for that UoS, or a student has received and exemption from the UoS coordinator.

Attendance at all lecture sessions is strongly recommended for all students.

Students who are unable to attend a compulsory class due to illness or misadventure must apply for special consideration (see policy on special consideration).

Notwithstanding the circumstances of absence or special consideration, students are expected to attend at least 85 % of the compulsory sessions in any UoS (unless otherwise stated in the syllabus document), failure to do so can result in failure of that UoS.

In cases where special consideration is accepted students may be required to attend an alternative class or placement, or undertake additional assessment tasks to fulfill the requirements of the UoS.

Tutors are expected to keep records of student attendance at compulsory sessions, and notify absences (especially when persistent or recurrent) to the relevant UoS coordinator in the first instance.

Final Year Additional Assessment Rule

Additional assessment may be permitted where a student in his or her final year has failed a single compulsory assessment resulting in a grade of fail in only that unit of study preventing him or her from completing the degree in that year.

For answers to Frequently Asked Questions to this rule please read the following document.

Final Year Rule FAQ (PDF 19k).

Criminal Record Clearance Card

NB: There are changes proposed for the criminal record clearance which should take effect on the 1st July 2009. From that date students will need to pay a fee and obtain a National Police Certificate but until then the procedures listed below remain in place.

All enrolled students are required to have a Criminal Record Clearance card which will need to be presented when clinical placements are done in NSW Health facilities or if requested by a preceptor. Students complete a consent form for a clearance card when enrolled in 1st year.

NSW Health Criminal Record Clearance Memo

USYD Criminal record Clearance Memo

If you happen to lose your clearance card or your details have changed and you require an amended clearance card, you must apply for a duplicate/amended card from NSW Health. A fee is charged for a duplicate/amended card and the request form must be signed by the course coordinator.

Duplicate/Amended card request form

If you have never been issued with a Criminal Record Clearance card, this will be confirmed with NSW Health before you can complete a consent form. Please note that a 100 point identity check is now required when completing a Criminal Record Check Consent Form. Forms are available at the Faculty Administration Office.

Vaccination Record Card

All students attending clinical placements in facilities operated by NSW Dept Health must have a documented vaccination history consistent with the requirements in the NSW Health Memo.

Please refer to the USYD Vaccination Memo for further information.

Students should note that vaccinations can take some time to complete and you should consult a doctor and commence your vaccinations at the start of your degree so that they are completed in time for your clinical placements/externships in the latter part of your degree.

Vaccination record cards are available from the Faculty Administration Office or the Student Centre.

Official University Communication

In order to ensure that the privacy and confidentiality of student information is maintained, electronic correspondence between staff and students should be restricted to the use of official university email accounts.

Where a student contacts a member of staff via a non-university email account, the staff member should respond to this email by requesting that the student re-send their communication from his or her official university email account. This policy extends to all email correspondence between a student and a staff member including those regarding special consideration.
