Exemptions for Repeating Students in Labs and Workshops

This policy applies to the following Units of Study:

  • PHAR1822
  • PHAR2811
  • PHAR2812
  • PHAR2813/PHAR3630
  • PHAR2823/PHAR3631
  • PHAR3602
  • PHAR3609
  • PHAR3610
  • PHAR4619

Students who are repeating a Unit of Study may be given an exemption from the laboratory/workshop component providing that they previously gained a minimum grade of credit in the laboratory/workshop component of that unit of study, (providing the laboratory/workshop component of a Unit of Study has not been changed significantly). Exemptions will not be given if the laboratory course was previously undertaken more than 3 years prior to the student repeating that Unit of Study.

A student must apply in writing (email) for an exemption to the laboratory/workshop component of a UoS. The UoS coordinator must notify the student of the outcome of their application for exemption in writing. Where the granting of an exemption results in a change to the weighting of the remaining components of that UoS the coordinator must also notify the student of this change in writing. The student must accept the offer of exemption in writing. The student must continue to attend all laboratory/workshop classes until an offer of exemption has been made and accepted in writing.

If an exemption is accepted then the student may not attend the workshop/practical class just for practice.

Criminal Record Clearance Card

NB: There are changes proposed for the criminal record clearance which will involve students needing to pay a fee and obtain a National Police Certificate. Until then the procedures listed below remain in place.

All enrolled students are required to have a Criminal Record Clearance card which will need to be presented when clinical placements are done in NSW Health facilities or if requested by a preceptor. Students complete a consent form for a clearance card when enrolled in 1st year.

NSW Health Criminal Record Clearance Memo

USYD Criminal record Clearance Memo

If you happen to lose your clearance card or your details have changed and you require an amended clearance card, you must apply for a duplicate/amended card from NSW Health. A fee is charged for a duplicate/amended card and the request form must be signed by the course coordinator.

Duplicate/Amended card request form

If you have never been issued with a Criminal Record Clearance card, this will be confirmed with NSW Health before you can complete a consent form. Please note that a 100 point identity check is now required when completing a Criminal Record Check Consent Form. Forms are available at the Faculty Administration Office.

Vaccination Record Card

All students attending clinical placements in facilities operated by NSW Dept Health must have a documented vaccination history consistent with the requirements in the NSW Health Memo.

Please refer to the USYD Vaccination Memo for further information.

Students should note that vaccinations can take some time to complete and you should consult a doctor and commence your vaccinations at the start of your degree so that they are completed in time for your clinical placements/externships in the latter part of your degree.

Vaccination record cards are available from the Faculty Administration Office or the Student Centre.

Attendance Policy

Attendance at Laboratory Classes, Tutorials and Clinical Placements is compulsory for all students unless it is explicitly stated in the syllabus document that attendance is optional for that UoS, or a student has received and exemption from the UoS coordinator.

Attendance at all lecture sessions is strongly recommended for all students.

Students who are unable to attend a compulsory class due to illness or misadventure must apply for special consideration (see policy on special consideration).

Notwithstanding the circumstances of absence or special consideration, students are expected to attend at least 85 % of the compulsory sessions in any UoS (unless otherwise stated in the syllabus document), failure to do so can result in failure of that UoS.

In cases where special consideration is accepted students may be required to attend an alternative class or placement, or undertake additional assessment tasks to fulfill the requirements of the UoS.

Tutors are expected to keep records of student attendance at compulsory sessions, and notify absences (especially when persistent or recurrent) to the relevant UoS coordinator in the first instance.

ALL written assessed work is to be submitted with an attached Compliance Statement. Work submitted without the compliance statement will NOT be marked.

All hard copies of assessments are to be submitted with an attached Compliance Statement via the Assessment/Assignment Box which is located in the reception area of the Faculty Administration Office and is only available during normal office hours (9.00am to 5.00pm - Monday to Friday).
